Please bring all previous medical records, relative x-rays, insurance information and a picture ID to your appointment.

If your insurance company requires a referral from a primary care physician and we do not have this information at the time of your appointment, or if you do not bring your x-ray or required record from a previous physician, we will need to reschedule your appointment.

In case of emergency outside of regular office hours, e.g., weekend, holiday, or late evening, please call the same office number and our answering service will contact the physician on call.
